70% of reengineering efforts fail. Reengineering methods work fine, if the organization is culturally able to implement them, if the appropriate knowledgeable people were brought into to process, if executive emphasis remained on the process, and if the methods were applied with discipline.
BPR is not something that can be engineered in project management software and them implemented like a machine process, but that's what many expect, especially executives.
BPR must be treated as a social and community planning/change process, and all of the political, social, emotional, and cultural issues must be managed as diligently, if not more so, that the technology or process issues.
You need a change management process, and lots of facilitated meetings, otherwise the human side of change will grind the technical side of change to a halt.
